*The appellant must provide precise details of his/her present state where assistance is needed. Every appeal should be accompanied by relevant details, facts and current status of all the previous attempts that are being made to address the issue at hand. The appellant must also provide his/her particulars while lodging appeal to Chairperson FAIDA Foundation and endorse his appeal through signatures or hand thumbs or as the case may be.
